Program Coordinator - LifeSharing

The Program Coordinator (Life Sharing) maintains both administrative and programmatic responsibilities to ensure program participants receive the needed services to live as independently as possible in the Lifesharing through Family Living service. This position fulfills the role of the Family Living Specialist under Chapter 6500 regulations for Family Living Homes, with a maximum caseload of 8 locations/16 individuals. Work is performed and reviewed under the direction of the Program Manager (Developmental Disability Services).
Program Coordinator $19/hr. - The essential function of the position includes the following:

  • Ensures programs are operated in accordance with principles of Inclusion, Positive Approaches and the principles elaborated in Everyday Lives.
  • Develops and implements individual assessments and individual support plans which include goals (formal and informal) and behavioral plans of support.
  • Provides assistance to the Lifesharing Sponsors in coordinating, scheduling and completing provisions for specialized services as identified in the individual habitation plan as individual needs occur, such as medical, dental, etc.
  • Assists Lifesharing Sponsors in counseling the program participant to deal more effectively with such matters as problem solving, interpersonal relationships, community responsibilities and civil rights to aid the program participant assume a more active role in family and community life.
  • Prepares quarterly reviews and annual assessments, and records observations in the behavioral status and progress of the program participant utilizing input from Lifesharing Sponsors.
  • Transports program participants to various locations, such as day program, doctor/clinic appointments, recreational activities, etc., when necessary.

Position Details:

  • Primarily Monday - Friday. Specific hours will flex based on needs of families and program participants.
  • Full- time status.
  • Must have knowledge and experience with crisis management and de-escalation.
  • Preferred knowledge of Trauma-Informed Care, including but not limited to Inclusion, increasing Resiliency, and system-based trauma.

All Team members must:

  • Must be 18 years or older.
  • Must have a valid, insurable drivers license.
  • Master's Degree and one year work experience working directly with persons with developmental disabilities; or Bachelor's Degree and two years work experience working directly with persons with developmental disabilities; or Associate's Degree/60 credit hours and two years work experience working directly with persons with developmental disabilities; or High School Degree/GED equivalent plus six years working directly with persons with developmental disabilities.
  • Must be registered, licensed or certified to practice their profession if that profession is governed by a registration, licensing or certification board in the Commonwealth of Pennsylvania.
