Psych Rehabilitation Counselor

  • Role models and implements psychosocial rehabilitation philosophy and principles while assisting individuals in their recovery inclusive of reestablishing roles in the community, developing support networks and increasing quality of life.
  •  Identifies individuals' strengths and needs.
  • Provides guidance, direction, support and monitoring of individuals during crises/emergencies and follows Agency procedures in providing assistance.
  •  Provides input and monitors individual's goals for developing community living skills.
  • Intervenes in individual's related problems and situations according to program procedures or supervisory direction.
  • Completes assigned shift duties and documents completion of duties on designated assignment sheets.
  •  Provides and facilitates psycho-educational individual and group/community meetings focusing on daily living skills and topics related to recovery.
  • Manages site operations including individuals' concerns and contacts during times in which other staff are absent.
  • Coordinates creative approaches to skills acquisitions for individuals.
  •  Provides services in a trauma-informed, resilience-oriented, equity-focused manner utilizing cultural sensitivity and person-centered planning.
  • Evaluates and documents individuals' progress.
  • Monitors and records all individual's medications during assigned shift in accordance with Step By Step CRR Medication Policy and Procedures.
  • Completes required documentation inclusive of staffing, case notes, DPNs, goal plan monitoring, incident reports, assignment sheets and other requested documentation.
  • Completes assessments in conjunction with the Residential Psychiatric Rehabilitation Caseworker's direction.
  •  Conducts and records fire drills as assigned.
  • Actively participates in planned activities in the site, in the community and within the Agency.
  • Recognizes, appreciates and incorporates the individual's culture into the rehabilitative plan to promote recovery.
  • When requested, coordinates and participates in recreational activities, both group and individual, at the site or in the community.
  • Adheres to Agency policies, procedures, Professional Standards and Guidelines.
  • Attends and participates in defined supervisory sessions as assigned by supervisor.
  • Encourages and utilizes input and feedback from individuals into program evaluation and service planning.
  • Participates in scheduled staff development activities.
  • Maintains clear, effective and professional communication with inter/intra-agency staff at all times during assigned shifts.
  • Attends and participates in assigned Agency/program committees and staff meetings.
  • Remains awake during all shifts.
  • Ensures the cleanliness and safety of the site, both inside and outside, while assisting and teaching individuals.
  • Provides drug and alcohol screenings.
  • Assumes the defined responsibilities for on-call as shift filler.
  • Other duties and tasks as assigned.

Center Street & Jefferson Street:  In addition to the above Essential Functions, Center Street & Jefferson Street Staff must also perform the following:

  • Provides support with cooking, shopping, cleaning, laundry and other daily activities.
  • Ensures transportation for individual's appointments, work and recreational activities when needed.
  • Assists individuals with daily hygiene activities when needed.
