Psych Rehabilitation Counselor - Lehigh and Northampton Counties

The Lehigh Valley has a Mobile Psych Rehab Counselor/Specialist position open at this time. The Psych Rehabilitation Counselor position is relied upon for ethical implementation of services and support to ensure quality services that are in accordance with all 5230 regulations. This position is a community-based position where the majority of the workday is out in the community with the individuals supported by the program.

  Mobile Psych Rehab Counselor $18/hr. - The essential function of the position includes the following:

  • Assist individuals with the recovery/rehabilitation process by engaging in informed and shared decision-making and facilitating partnerships with other persons identified by the individual receiving services. Provides all services based on the strengths and capabilities of individuals and ensures all services are person-centered meeting the unique needs, values, hopes and aspirations of each person.
  • Completes an assessment in collaboration with the individual prior to developing an Individual Rehabilitation Plan (IRP).
  • Identifies the functioning of an individual in the living, learning, working and socializing domains.
  • Helps individuals identify existing and needed natural and formal supports, including peer support initiatives, self-and-mutual-help groups, health care facilities and social service agencies.
  • Jointly develops an IRP with individuals that is consistent with the assessment by day 20 of service, but no more than 60 calendar days after initial contact.
  • Updates IRP at least every 90 calendar days or when the individual requests a change, or when the individual completes a goal or when the individual has not made significant progress on a goal.
  • Completes written and/or electronic documentation for each day a service was provided which includes date, time, duration, location, type of interaction, service provided in context of the goal, and the individual's response to service.
  • Assists individual with identifying next steps and resources, and makes any necessary referrals.
  • Completes a discharge summary within 30 days.
  • All Team members must:

    • Must be 18 years or older.
    • Must have a valid, insurable drivers license.
    • Bachelor's degree, OR Associate degree and one-year work experience in mental health direct service, OR A Certified Peer Specialist (CPS) certificate and one-year paid or volunteer work experience in mental health direct service, OR A high school diploma or GED and two-years work experience in human services which must include one year of mental health direct service.
    • Must be registered, licensed or certified to practice their profession if that profession is governed by a registration, licensing or certification board in the Commonwealth of Pennsylvania.
    • have no prohibited criminal offenses.

    • Mobile Psych Rehab Specialist $20/hr. - The essential function of the position are the same as the Mobile Psych Rehab Counselor except this position must maintain their CPRP (Certified Psychiatric Rehabilitation Practitioner).

        All Team members must:

    • Must have a valid, insurable drivers license.
    • Bachelor's Degree and 2 years work experience in mental health direct service, 1 year of which must be work experience in Psychiatric Rehabilitation Service (PRS). Certified Psychiatric Rehabilitation Practitioner (CPRP) certification shall be attained within 2 years from the date of hire as a psychiatric rehabilitation specialist ; or CPRP certification
    • Must be registered, licensed or certified to practice their profession if that profession is governed by a registration, licensing or certification board in the Commonwealth of Pennsylvania.
    • have no prohibited criminal offenses

    Position Details:

    • Primarily Monday - Friday. Specific hours will flex based on needs of program participants.
    • Full- time status.
    • Must have knowledge and experience with crisis management and de-escalation.
    • Preferred knowledge of Trauma-Informed Care, including but not limited to Inclusion, increasing Resiliency, and system-based trauma.
