ESSENTIAL FUNCTIONS

Ensures efficiency and effectiveness in programming for assigned consumers through the completion of specific tasks and the provision of input towards the following:

nIdentification of consumer's strengths and/or needs.

nIdentification of skill areas which may need to be developed or improved.

nEstablishment of individual consumer goals for developing community living skills.

nImplementation of methods, processes, and plans (goal planning, supportive counseling, guidance, group activities, tutoring, role modeling, experiential opportunities, etc.) to address specific program areas, e.g. meal planning, shopping, leisure activities, etc.

nEvaluates consumer's progress.

nEstablishes and maintains clear communications with intra-agency staff.

nAttends and participates in defined supervisory sessions, case staffings, information meetings, etc.
